项目概况 工程名称：大团镇2026年村卫生室中医阁建设 1、 项目预算：工程总投资约 64万元 2、 设计预算：2.8万元。 3、 工程地点：车站村、果园村、邵村村、扶栏村和金园村共 5个村卫生室 4、 工程内容：对村卫生室室内部分设施进行改建，改建涉及室内隔墙、水电、吊顶、地面等。 二、采购目标 1、对本项目的设计进行采购 三、供应商要求 资质要求：房屋建筑工程资质（乙级及以上，包含综合资质），并在人员组成结构等方面具有相应的设计能力。 工期：合同签订之日起至项目竣工结算。 服务内容：承担本项目的方案设计、施工图设计。 成果文件：方案文件、施工蓝图。 设计标准：本工程设计过程和成果必须符合国家有关工程建设标准强制性条文和现行的标准、规范。 四、本项目选派方式为竟价选取

（四）在中介集市报名后中介服务合同签订前，存在记入一般或者严重不良诚信档案行为的

I. One recorded minor bad credit behavior within 3 months before registration in the intermediary market.

II. Two recorded minor bad credit behaviors within 1 year before registration in the intermediary market.

III. One recorded serious bad credit behavior within 2 years before registration in the intermediary market.

IV. Any recorded general or serious bad credit behavior between the registration in the intermediary market and before the signing of the intermediary service contract.
