一、项目概况 1、 工程名称：大团镇2026年度村卫生室中医阁建设项目 2、 项目预算：工程总投资约 64万元， 3、 监理预算： 2.3万元。 4、 工程地点：车站村、果园村、邵村村、扶栏村和金园村共5个村卫生室。 5、 工程计划：计划 2026 年 5月实施。 6、 计划工期：45天，具体实际为准。 二、 采购目标 1、资质要求：房屋建筑工程资质（乙级及以上，包含综合资质） 2、监理内容：消防改造工程项目的质量、进度、费用控制，安全生产文明施工的监督管理，合同、信息等方面的协调管理等内容。 3、回避的情形： 1) 参与同一项目采购的不同中介服务机构法定代表人或实际控制人为 同一人的； 2) 中介服务机构的法定代表人或实际控制人和委托人单位法定代表人或者负责人有配偶、直系血亲（含拟制血亲）、三代以内旁系血亲或者近姻亲关系的； 3) 其他应当回避的情形。 4、否定性条件： 1) 在中介集市报名前三个月内存在1次记入一般不良诚信档案行为的； 2) 在中介集市报名前1年内存在2次记入一般不良诚信档案行为的； 3) 在中介集市报名前2年内存在1次记入严重不良诚信档案行为的； 4) 在中介集市报名后中介服务合同签订前，存在记入一般或者严重不良诚信档案行为的。 5、中选办法：委托人根据综合能力及报价均能满足竞价文件实质性响应要求且择优的原则确定中选服务商。 三、上报文件组成 1、 营业执照扫描件 2、 资质证书扫描件

（四）在中介集市报名后中介服务合同签订前，存在记入一般或者严重不良诚信档案行为的

I. One recorded minor bad credit behavior within 3 months before registration in the intermediary market.

II. Two recorded minor bad credit behaviors within 1 year before registration in the intermediary market.

III. One recorded serious bad credit behavior within 2 years before registration in the intermediary market.

IV. Any recorded general or serious bad credit behavior between the registration in the intermediary market and before the signing of the intermediary service contract.
